Community Health Centers -- Providing A Base Of Care

Under the Affordable Care Act, millions of new patients will have access to the health care system. One concern – where will all of these people obtain their health care? Because of fee for service and the inequitable tilt toward specialty care, the primary care system in our country has been decimated and will take years to rebuild.

Fortunately, the ACA puts a lot of money and effort toward correcting this imbalance. But even with all the dollars in the world, it would take many years to repair decades of damage.  The section of the ACA intended to repair our primary care system thankfully started rolling out right after the law was passed.  Primary care physicians are working hard to become certified medical care homes, which expands the role of staff and ancillary providers to pick up the workload of important but mundane tasks that do not require the work of a physician brain.  This will increase capacity, but there is still so much that has to be done to handle the new patients coming into the system.

There is hope for the newly insured. One gem of our health care system is the vast network of community health centers.  Community health centers as we know them today started in 1965 to provide health care access to poor and medically underserved communities.  Today, according to the National Association of Community Health Centers, greater than 20 million patients are served in over 8,000 locations around the country.  These groups provide care for anyone – those without insurance pay on a sliding scale. They also take Medicaid and private insurance.

Although not widely stated, it was expected that the majority of new patients entering the health care system would be served by community health centers.  With the initial funding provided by the ACA, these centers were set to expand their capability to 40 million patients by 2015 – that is double their current number! Unfortunately, base funding for these centers was significantly cut by both the states and the federal government – they put some money in one hand and took it out of the other.  Because of this, community health centers pared their projections back to serving 30 million people by 2015.

Being simplistic, which is always my goal, of the estimated 30 million new people who will enter the entire health care system, 20 million would have been seen in the community health centers and the other 10 million in private physician offices.  Now those numbers will have to flip flop, because the decreased funding of the community health centers will only allow for 10 million more patients then they are currently serving.

This is a shame. Unfortunately, the decrease in funding is shooting our country in the foot.  Why? Community health centers are very efficient at providing primary care and preventive care.  Nationally, it costs $610 per year to provide comprehensive primary care to a patient in community health centers and $964 per year in other physician settings. This would save $354 per patient per year over the 10 million patients who now have to be seen by the private sector – that could have been a total savings of $3.54 billion per year.  This doesn’t take into account the added savings of keeping people healthy and out of emergency departments and hospitals. To put this into perspective, the total federal base budget for community health centers was $2 billion per year before funding was cut by $600 million last year.  States also cut funding by about another $200 million per year. So basically, we cut $800 million per year between the two and could have saved $3.54 billion per year? My financial planner brain cannot wrap itself around that one. Am I missing something?

There is another twist to this sordid tale. The insurance breakdown of patients seen in community health centers is about 40% uninsured and 40% on Medicaid, with the remaining 20% on other insurance. About 80% of patients seen at community health centers are under 133% poverty level and would have qualified for Medicaid under the Affordable Care Act.  The community health centers were excited about this – their funding would improve and not be at the whim of state and federal politicians. Governors in six states so far are refusing the federal dollars to expand Medicaid, so the community health centers in those states will still have to spend significant time and energy lobbying their legislatures for money. This seems like a counterproductive waste of time.

Some states are also refusing other funding allocated by the ACA. I spoke with Andy Behrman, President and CEO of the Florida Association of Community Health Centers,  regarding the challenges in his state. Florida has 48 community health centers with over 320 sites that provide care for 1.17 million patients. Eight of the centers rely on state funding – so they are at the mercy of the state politicians. Governor Scott has refused ACA funding to expand and improve Florida’s eight state funded community health centers. Florida community health centers are very efficient – it costs $510 per person per year to provide comprehensive primary care including lab and x-ray services.   Thank goodness only eight of the 48 centers rely on Governor Scott’s whims.

A strong health care system is important for our country’s resiliency and is a matter of national defense. It would be great if we could cut the $500 billion per year worth of wasted overhead from our health care system, and divert just a couple of billion per year to community health centers.  We would be much better for it.

Community Health Systems Raising $1 Billion With Eight-Year Debt

Community Health Systems Inc. (CYH), the second-largest U.S. hospital chain, plans to raise $1 billion to refinance notes maturing in 2015.

Proceeds from the new senior debentures due in 2020 will be used to purchase the 8.875 percent bonds at 102.6 cents on the dollar after a July 3 tender offer, the Franklin, Tennessee- based company said today in a regulatory filing. The 2015 notes, rated six levels below investment grade at B3 by Moody’s Investors Service, traded at 102.7 cents to yield 7.86 percent at 8:22 a.m. in New York, according to Trace, the bond-price reporting system of the Financial Industry Regulatory Authority.

Community Health Systems is selling the debt less than two weeks after the U.S. Supreme Court decided to leave President Barack Obama’s transformation of the health system substantially intact. Junk bonds of health-care providers rallied after the ruling, which boosted investor optimism for the industry by reducing the risk that the companies will be stuck with unpaid bills.

Credit Suisse Group AG, Bank of America Corp., Citigroup Inc., Credit Agricole SA, Goldman Sachs Group Inc., JPMorgan Chase & Co., Morgan Stanley, Royal Bank of Canada, SunTrust Banks Inc. and Wells Fargo & Co. are managing the sale, according to the filing.

Community Health Systems Announces Acquisition of York, Pennsylvania Hospital

FRANKLIN, Tenn.--(BUSINESS WIRE)--

Community Health Systems, Inc. (CYH) announced today that subsidiaries of the Company have acquired substantially all of the assets of Memorial Health Systems in York, Pennsylvania. The system includes 100-bed Memorial Hospital, the Surgical Center of York, and other outpatient and ancillary services. Memorial Hospital becomes the seventeenth Community Health Systems, Inc. affiliated hospital in Pennsylvania. The purchase agreement includes a commitment to construct a replacement hospital within five years of the closing date.

SCAN Health Plan Honored for Excellence in Community Outreach

LONG BEACH, Calif.--(BUSINESS WIRE)--

SCAN Health Plan was one of eight healthcare organizations honored by the Los Angeles Business Journal (LABJ) for their outstanding efforts in serving the community. Community Health Systems, Inc. Announces $300 Million Receivables Securitization Program

FRANKLIN, Tenn.--(BUSINESS WIRE)--

Community Health Systems, Inc. (the “Company”) (NYSE: CYH - News) announced today that its wholly-owned subsidiary, CHS/Community Health Systems, Inc. (“CHS”), and certain of CHS’ subsidiaries have entered into an accounts receivable securitization program (the “Receivables Facility”) with a group of conduit lenders and liquidity banks (the “Lenders”). Credit Agricolé Corporate and Investment Bank will serve as managing agent and the administrative agent and The Bank of Nova Scotia will also serve as managing agent. During the term of the Receivables Facility, these subsidiaries of CHS will sell all existing and future accounts receivable (the “Receivables”) to CHS, which will then sell or contribute the Receivables to CHS Receivables Funding, LLC, a bankruptcy-remote, special-purpose limited liability company. CHS Receivables Funding, LLC will in turn grant security interests in the Receivables to Credit Agricolé Corporate and Investment Bank in exchange for borrowings from the Lenders of up to $300 million outstanding from time to time based on the availability of eligible Receivables and other customary factors. Unless earlier terminated or subsequently extended pursuant to its terms, the Receivables Facility will expire on March 21, 2014, subject to customary termination events that could cause an early termination date.

The Receivables Facility will be more fully described in, and the principal agreements establishing the program will be filed as exhibits to, a Current Report on Form 8-K to be filed with the Securities and Exchange Commission.

Community Health Systems, Inc. Prevails in Tenet Litigation

Community Health Systems, Inc. (the “Company”) (NYSE: CYH - News) announced today that it has prevailed completely in the litigation Tenet Healthcare Corporation filed against the Company. United States District Court Judge Barbara M. G. Lynn dismissed with prejudice Tenet Healthcare Corporation vs. Community Health Systems, Inc., Wayne T. Smith and W. Larry Cash, citing Tenet’s lack of standing to bring this action.

Smith, chairman, president and chief executive officer of Community Health Systems, Inc., said, “We believe Tenet’s Board and management team initiated this litigation for the purpose of distracting shareholders from our bid to buy their company. National Community Health Center Partnership Forms to Improve Health IT Services

Community Health Systems Announces Affiliation with Longview, Texas Multi-Specialty Clinic

(NYSE: CYH - News) announced today that a subsidiary of the Company has merged operations with Diagnostic Clinic of Longview, P.A., a large multi-specialty clinic serving residents of Longview, Texas, and surrounding East Texas communities. More than 100 physicians, 30 mid-level providers and 500 other employees of the practice provide quality care at 18 area locations. In accordance with Texas law, the physicians of Diagnostic Clinic of Longview are employed by a separate entity.

Community Health Systems Announces Acquisition of Blue Island, Illinois Hospital

Community Health Systems, Inc. (NYSE: CYH - News) announced today that a subsidiary has acquired substantially all of the assets of MetroSouth Medical Center in Blue Island, Illinois. Located approximately 20 miles south of Chicago, the 330-bed hospital offers a full spectrum of healthcare services. Community Health Systems Announces Acquisition of Blue Island, Illinois Hospital

FRANKLIN, Tenn.--(BUSINESS WIRE)--

Community Health Systems, Inc. (NYSE: CYH - News) announced today that a subsidiary has acquired substantially all of the assets of MetroSouth Medical Center in Blue Island, Illinois. Located approximately 20 miles south of Chicago, the 330-bed hospital offers a full spectrum of healthcare services. MetroSouth Medical Center becomes the ninth Community Health Systems, Inc. affiliated hospital in Illinois and the third one serving communities around the greater Chicago region.

Myca Health Adds New Primary Care Clients Serving 10,000 Patients to its Hello Health® Community

NEW YORK, Jan. 25, 2012 /PRNewswire/ -- Myca Health Inc. announced today that in the past month primary care practices serving over 10,000 patients have contracted to implement the Hello Health® Patient Management platform. The company recently signed agreements with family practice, internal medicine, pediatric and integrative medicine practices in five states. Community Health Systems Announces Definitive Agreement to Acquire York, Pennsylvania Hospital

FRANKLIN, Tenn.--(BUSINESS WIRE)-- Community Health Systems, Inc. (NYSE: CYH - News) announced today that subsidiaries of the Company have executed a definitive agreement to acquire substantially all of the assets of Memorial Health Systems in York, Pennsylvania. Located in one of the fastest growing counties in Pennsylvania, the health system includes 100-bed Memorial Hospital, the Surgical Center of York, and other outpatient and ancillary services. The agreement includes a commitment to construct a replacement hospital within five years of the closing date. The transaction is subject to customary federal and state regulatory approvals, including review and approval by the Attorney General of the Commonwealth of Pennsylvania.

Health Net Community Solutions Centers Offer Free Health and Wellness Activities to Local Communities They Serve
